Uses
Indicated for the treatment of hypertension (extended-release formulation) and chronic stable angina. May be used as monotherapy or in combination with other antihypertensive agents.
Do not use if
Contraindicated with concomitant use of strong CYP3A4 inducers (e.g., rifampin), in cardiogenic shock, and in patients with known hypersensitivity to nifedipine or any component of the formulation.
Side effects
Common side effects include peripheral edema, flushing, headache, dizziness, and palpitations. Excessive hypotension may occur, especially at initiation or with dose escalation.
Warnings
Excessive hypotension may occur during initial therapy or dose escalation; use with caution in patients with heart failure or hemodynamic instability. Avoid abrupt discontinuation. Use with caution in patients with hepatic impairment as nifedipine is metabolized by CYP3A4.
How it works
Nifedipine is a dihydropyridine calcium channel blocker that inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle cells. This reduces peripheral vascular resistance and lowers blood pressure, and in angina it reduces myocardial oxygen demand by decreasing afterload.
Storage
Store at controlled room temperature, 15°C–30°C (59°F–86°F). Protect from light and moisture. Keep tablets in original container.
