Uses

A synthetic progestogen indicated for conditions requiring progesterone support including endometriosis, dysmenorrhea, irregular menstrual cycles, premenstrual syndrome, threatened or recurrent miscarriage, infertility due to luteal insufficiency, and hormone replacement therapy as the progestogenic component.

Do not use if

Contraindicated in known or suspected progesterone-dependent tumors, undiagnosed vaginal bleeding, severe hepatic impairment, and hypersensitivity to dydrogesterone. Use with caution in patients with a history of depression, epilepsy, or cardiovascular disease.

Side effects

Generally well tolerated. May cause headache, nausea, breast tenderness, irregular bleeding, and mood changes. Unlike testosterone-derived progestogens, it has no androgenic, estrogenic, or glucocorticoid activity, reducing related side effects.

Warnings

Patients with a history of depression should be monitored closely during treatment. Any breakthrough bleeding or unexpected vaginal bleeding should be investigated. Discontinue if signs of hepatic dysfunction occur. Pregnancy should be excluded before initiating treatment for irregular bleeding.

How it works

Dydrogesterone is a retroprogesterone (stereoisomer of progesterone) that selectively binds to progesterone receptors with high affinity. It induces secretory transformation of the endometrium and has no estrogenic, androgenic, anabolic, or glucocorticoid effects, making it a highly selective progestogen.
