Uses
Indicated for the treatment of schizophrenia in adults and adolescents (13–17 years). Also indicated for the treatment of major depressive episodes associated with bipolar I disorder (bipolar depression) as monotherapy in adults and pediatric patients (10–17 years), and as adjunctive therapy with lithium or valproate in adults.
Do not use if
Contraindicated in patients with known hypersensitivity to lurasidone or any component of the formulation. Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) or strong CYP3A4 inducers (e.g., rifampin, St. John's wort) is contraindicated.
Side effects
Common adverse reactions include somnolence, akathisia, nausea, parkinsonism, and agitation. Serious risks include tardive dyskinesia, neuroleptic malignant syndrome, increased mortality in elderly patients with dementia-related psychosis, and suicidal thoughts in younger patients.
Warnings
Elderly patients with dementia-related psychosis treated with antipsychotics have an increased risk of death. Increased risk of suicidal thinking in children and young adults. Monitor for metabolic changes, orthostatic hypotension, and seizures. Not approved for dementia-related psychosis.
How it works
Lurasidone is an atypical antipsychotic that acts as an antagonist at dopamine D2 and serotonin 5-HT2A receptors, with additional high-affinity antagonism at 5-HT7 and partial agonism at 5-HT1A receptors. This receptor profile contributes to both antipsychotic and antidepressant effects.
