Uses

This combination is indicated for the relief of migraine headache and associated nausea. Metoclopramide acts as a prokinetic antiemetic that also enhances gastric emptying, thereby improving absorption of paracetamol. Paracetamol provides analgesic and antipyretic effects.

Do not use if

Contraindicated in patients with pheochromocytoma, epilepsy, gastrointestinal hemorrhage, mechanical obstruction, or perforation. Avoid in patients with a history of tardive dyskinesia from dopamine antagonists. Paracetamol is contraindicated in severe hepatic impairment.

Side effects

Metoclopramide may cause drowsiness, restlessness, extrapyramidal reactions (including acute dystonia), and rarely tardive dyskinesia with prolonged use. Paracetamol is generally well tolerated but can cause hepatotoxicity in overdose. Combined use may increase sedation.

Warnings

Use with caution in elderly patients due to increased risk of extrapyramidal effects. Avoid in patients with Parkinson's disease. Do not exceed the recommended dose of paracetamol. Patients should avoid concurrent use of other paracetamol-containing products to prevent overdose.

How it works

Metoclopramide is a dopamine D2 receptor antagonist and 5-HT4 agonist that accelerates gastric emptying and has central antiemetic effects at the chemoreceptor trigger zone. Paracetamol inhibits prostaglandin synthesis in the central nervous system, producing analgesia and antipyresis.
