Uses

Ambrisentan is indicated for the treatment of pulmonary arterial hypertension (PAH, WHO Group 1) in adults to improve exercise capacity and delay clinical worsening. It is primarily studied in patients with WHO Functional Class II–III symptoms, including idiopathic, heritable, and connective tissue disease-associated PAH.

Do not use if

Contraindicated in pregnancy due to confirmed teratogenicity in animal studies. Also contraindicated in patients with idiopathic pulmonary fibrosis.

Side effects

Common adverse effects include peripheral edema, fluid retention, nasal congestion, headache, palpitations, and flushing. Decreased sperm count, anemia (hemoglobin reduction), and pulmonary edema in patients with pulmonary veno-occlusive disease have also been reported.

Warnings

Carries a boxed warning for embryo-fetal toxicity; females of reproductive potential must use effective contraception and enroll in a REMS program. Monitor for fluid retention and signs of pulmonary edema. Periodic hemoglobin monitoring is recommended.

How it works

Ambrisentan is a selective endothelin type-A (ET-A) receptor antagonist. By blocking ET-A receptors on vascular smooth muscle, it inhibits the vasoconstrictive and proliferative effects of endothelin-1, thereby reducing pulmonary vascular resistance and improving hemodynamics.

Storage

Store at 20°C to 25°C (68°F to 77°F) with excursions permitted between 15°C and 30°C. Keep in original packaging.
