Uses

This combination is used for treatment of epilepsy (generalized, partial, and absence seizures) and as prophylaxis for migraine. It is also used as a mood stabilizer in bipolar disorder.

Do not use if

Contraindicated in known hepatic disease or significant hepatic dysfunction, mitochondrial disorders caused by POLG mutations, and known hypersensitivity to valproate. Absolutely contraindicated in pregnancy due to teratogenicity.

Side effects

Common side effects include nausea, tremor, weight gain, hair loss, and drowsiness. Serious risks include hepatotoxicity, pancreatitis, thrombocytopenia, and hyperammonemic encephalopathy.

Warnings

High risk of fetal harm (neural tube defects, cognitive impairment in offspring); females of childbearing potential must use effective contraception. Monitor liver function and complete blood count regularly. Risk of suicidal ideation as with all antiepileptics.

How it works

Valproic acid and its sodium salt increase GABA activity in the brain by inhibiting GABA transaminase and blocking voltage-gated sodium and calcium channels. This broad spectrum of action underlies its use across multiple seizure types.
