Uses

Indicated for controlling manifestations of clinical spasticity from upper motor neuron disorders such as spinal cord injury, stroke, cerebral palsy, and multiple sclerosis. Also used intravenously for the treatment and prevention of malignant hyperthermia. Intended for patients whose rehabilitation is impaired by spasticity, where relief may restore residual function.

Do not use if

Contraindicated in active hepatic disease such as hepatitis or cirrhosis. Also contraindicated when spasticity is used by the patient to maintain upright posture, balance during locomotion, or to sustain increased function.

Side effects

Most common side effects include drowsiness, dizziness, weakness, general malaise, fatigue, and diarrhea. These are typically transient and occur early in treatment. Diarrhea may be severe enough to require temporary dose reduction. Hepatotoxicity, including fatal cases, has been reported.

Warnings

Carries a significant risk of hepatotoxicity including fatal hepatitis; liver function should be monitored before and during treatment. Risk increases with higher doses and prolonged use. Should not be used for conditions other than those specifically recommended.

How it works

Dantrolene acts directly on skeletal muscle beyond the neuromuscular junction. It dissociates excitation-contraction coupling by interfering with calcium release from the sarcoplasmic reticulum via the ryanodine receptor (RyR1), thereby reducing muscle contractility without blocking nerve conduction.
