Uses

Indicated as a prophylactic agent to reduce the incidence of hemorrhagic cystitis induced by ifosfamide chemotherapy. It is not indicated for hematuria caused by other conditions.

Do not use if

Contraindicated in patients with known hypersensitivity to mesna or to any of the excipients including benzyl alcohol.

Side effects

Common side effects include nausea, vomiting, and diarrhea. Serious risks include hypersensitivity reactions including anaphylaxis, and dermatological toxicity such as skin rash with eosinophilia.

Warnings

Anaphylactic reactions have been reported; monitor patients closely and discontinue if severe hypersensitivity occurs. Dermatologic toxicity including Stevens-Johnson syndrome has been reported rarely.

How it works

Mesna is a thiol compound that reacts chemically with urotoxic metabolites of ifosfamide (acrolein and 4-hydroxy-ifosfamide) in the renal tubules and bladder, converting them to non-toxic compounds and protecting the urothelium.

Storage

Store at controlled room temperature, protected from light. Opened multidose vials may be stored and used for up to 8 days.
