Uses

This combination of amlodipine (a dihydropyridine calcium channel blocker) and bisoprolol (a cardioselective beta-1 blocker) is indicated for the management of hypertension and stable angina pectoris, particularly in patients requiring both drug classes for optimal blood pressure and heart rate control.

Do not use if

Contraindicated in cardiogenic shock, decompensated heart failure, sick sinus syndrome, high-degree AV block (without pacemaker), severe bradycardia, and hypersensitivity to either component. Avoid in patients with severe asthma or COPD.

Side effects

Common side effects include edema (mainly ankle swelling from amlodipine), fatigue, bradycardia, dizziness, and headache. Cold extremities and bronchospasm may occur due to the beta-blocker component.

Warnings

Do not abruptly discontinue bisoprolol; taper dose gradually to avoid rebound hypertension or angina. Monitor for signs of heart failure. Use with caution in diabetes (may mask hypoglycemia symptoms). Avoid in severe peripheral arterial disease.

How it works

Amlodipine blocks L-type voltage-gated calcium channels in vascular smooth muscle and cardiac cells, causing vasodilation and reduced cardiac afterload. Bisoprolol competitively blocks beta-1 adrenoceptors, reducing heart rate and myocardial contractility, thereby lowering blood pressure and myocardial oxygen demand.
