Uses

Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. Not for use in type 1 diabetes or diabetic ketoacidosis.

Do not use if

Contraindicated in patients with a history of hypersensitivity to nateglinide or its inactive ingredients.

Side effects

Hypoglycemia is the primary risk. Common adverse reactions (incidence ≥3%) include upper respiratory tract infection, back pain, flu symptoms, dizziness, arthropathy, and diarrhea.

Warnings

Administer immediately before meals; skip the dose if a meal is skipped to reduce hypoglycemia risk. No established evidence of cardiovascular benefit in reducing macrovascular outcomes. Use with caution in hepatic impairment.

How it works

Nateglinide is a glinide (phenylalanine derivative) that stimulates insulin secretion from pancreatic beta cells by blocking ATP-sensitive potassium channels, causing membrane depolarization and calcium influx. It has a rapid onset and short duration, targeting postprandial hyperglycemia.
