Uses

Indicated for the treatment of pain and inflammation associated with cataract surgery. Applied topically to the eye as a prodrug that is converted to amfenac within ocular tissues.

Do not use if

Contraindicated in patients with known hypersensitivity to nepafenac or any component of the formulation, or to other NSAIDs. Cross-sensitivity with aspirin and other NSAIDs is possible.

Side effects

Common adverse reactions (5–10%) include capsular opacity, decreased visual acuity, foreign body sensation, increased intraocular pressure, and sticky sensation in the eye. Rare cases of corneal thinning or perforation have been reported.

Warnings

Use with caution in patients with known bleeding tendencies or those on anticoagulant therapy, as NSAIDs may increase bleeding time. Topical NSAIDs may cause delayed corneal healing; discontinue if corneal epithelial breakdown occurs. Avoid use in contact lens wearers.

How it works

Nepafenac is a prodrug that penetrates the cornea and is hydrolyzed by ocular tissue hydrolases to amfenac, which inhibits prostaglandin H synthase (COX-1 and COX-2), thereby reducing prostaglandin synthesis and ocular inflammation.

Storage

Store at controlled room temperature 15–25°C. Protect from light. Shake well before use.
