Uses

The combination of amlodipine (a dihydropyridine calcium channel blocker) and indapamide (a thiazide-like diuretic) is indicated for the treatment of hypertension in adults, particularly when monotherapy is insufficient. The combination lowers blood pressure through complementary mechanisms, reducing cardiovascular risk.

Do not use if

Contraindicated in anuria, severe hepatic impairment, severe renal impairment, hypokalemia refractory to treatment, hypersensitivity to either component or to sulfonamide-derived drugs, and during pregnancy. Indapamide is contraindicated in severe liver disease.

Side effects

Common adverse effects include peripheral edema (from amlodipine), hypokalemia, hyponatremia, and hyperuricemia (from indapamide), headache, dizziness, and flushing. Electrolyte imbalances and glucose disturbances may occur with indapamide.

Warnings

Monitor electrolytes (especially potassium) and renal function during treatment. Indapamide may exacerbate gout or hyperuricemia. Use with caution in patients with diabetes. Do not use during pregnancy due to fetal risk from the diuretic and calcium channel blocker components.

How it works

Amlodipine blocks L-type calcium channels in vascular smooth muscle, causing vasodilation and reduced peripheral resistance. Indapamide inhibits sodium and chloride reabsorption in the distal tubule, promoting sodium and water excretion and reducing plasma volume. Together they provide additive antihypertensive effects.
