Uses

An anticholinergic agent used for the management of Parkinson's disease and drug-induced extrapyramidal symptoms including tremor, rigidity, and akathisia caused by antipsychotic medications.

Do not use if

Contraindicated in glaucoma, urinary retention, prostatic hypertrophy, myasthenia gravis, and bowel obstruction. Use with caution in cardiovascular disease.

Side effects

Common anticholinergic effects include dry mouth, blurred vision, urinary retention, constipation, tachycardia, and confusion, especially in the elderly. High doses may cause hallucinations and agitation.

Warnings

Use with extreme caution in elderly patients due to risk of cognitive impairment and urinary retention. Abrupt withdrawal after prolonged use may worsen symptoms. Avoid in hot environments as it impairs sweating.

How it works

Procyclidine is a muscarinic acetylcholine receptor antagonist. It reduces cholinergic excess in the basal ganglia that arises from dopamine deficiency in Parkinson's disease, thereby alleviating tremor and muscle rigidity. It restores the dopaminergic-cholinergic balance in the striatum.
