Uses

Fosinopril, an ACE inhibitor, is indicated for the treatment of hypertension, either alone or in combination with other antihypertensive agents. It is also used in the management of heart failure as adjunctive therapy.

Do not use if

Contraindicated in patients with a history of ACE inhibitor-associated angioedema, bilateral renal artery stenosis, pregnancy (all trimesters), and in combination with aliskiren in patients with diabetes or renal impairment. Contraindicated with sacubitril/valsartan.

Side effects

Common adverse effects include a dry persistent cough, dizziness, hypotension (especially first-dose), hyperkalemia, and fatigue. Angioedema, though rare, is a serious adverse reaction that requires immediate discontinuation.

Warnings

Fetal toxicity: discontinue immediately upon detection of pregnancy. Monitor renal function, potassium levels, and blood pressure during therapy. Risk of neutropenia exists, particularly in patients with renal impairment or collagen-vascular disease.

How it works

Fosinopril is a prodrug hydrolyzed to fosinoprilat, an active ACE inhibitor. Inhibition of ACE reduces conversion of angiotensin I to angiotensin II, decreasing vasoconstriction and aldosterone secretion, resulting in lowered blood pressure and reduced cardiac preload and afterload.

Storage

Store at room temperature between 20–25°C, protected from moisture. Keep in tightly closed container out of reach of children.
