Uses

Methylergometrine (methylergonovine) maleate is an oxytocic agent indicated for the prevention and treatment of postpartum hemorrhage and subinvolution of the uterus due to uterine atony. It is used to promote uterine contraction after delivery of the placenta.

Do not use if

Contraindicated in hypertension, preeclampsia/eclampsia, peripheral vascular disease, coronary artery disease, and in the first and second stages of labor. Contraindicated in patients with known hypersensitivity to ergot alkaloids.

Side effects

Common adverse effects include nausea, vomiting, and elevated blood pressure (hypertension). Severe reactions may include coronary artery spasm, myocardial infarction, stroke, and ergotism with prolonged use.

Warnings

Use with extreme caution in patients with hypertension, sepsis, or cardiovascular disease. Intravenous administration may cause severe hypertensive episodes and should only be used in emergencies. Do not administer before delivery of the placenta to avoid fetal hypoxia.

How it works

Methylergometrine is a semisynthetic ergot alkaloid that acts directly on uterine smooth muscle, causing sustained tonic contraction. It acts as a partial agonist at alpha-adrenergic, serotonergic (5-HT2), and dopaminergic receptors. The uterotonic effect reduces blood loss from the placental implantation site.

Storage

Store at 2–8°C (refrigerated). Protect from light. Oral tablets may be stored at room temperature.
