Uses
Indicated for the treatment of hypertension. Particularly useful in the management of hypertension during pregnancy (one of the preferred antihypertensives in pregnancy).
Do not use if
Contraindicated in patients with active hepatic disease (acute hepatitis, active cirrhosis), history of methyldopa-related liver disorders, hypersensitivity to any component, and in patients receiving MAO inhibitor therapy.
Side effects
Common side effects include sedation, fatigue, headache, and dizziness, especially at the start of therapy. May cause dry mouth, bradycardia, hepatotoxicity, and a positive Coombs test with potential hemolytic anemia.
Warnings
Monitor liver function periodically; discontinue if signs of hepatic damage appear. Hemolytic anemia may occur; monitor blood counts. Positive Coombs test is common but does not always indicate hemolysis. Use with caution in renal impairment.
How it works
Methyldopa is a centrally acting antihypertensive that is metabolized to alpha-methylnorepinephrine, which stimulates central inhibitory alpha-2 adrenergic receptors in the brainstem, reducing sympathetic outflow and thereby lowering peripheral vascular resistance and blood pressure.
