Uses

This topical combination of erythromycin (antibiotic) and isotretinoin (retinoid) is indicated for the treatment of acne vulgaris. Erythromycin reduces Cutibacterium acnes colonization and inflammation, while isotretinoin normalizes keratinocyte differentiation and reduces sebum production locally.

Do not use if

Contraindicated in patients with known hypersensitivity to erythromycin, isotretinoin, or other retinoids. Should not be used in pregnancy given the teratogenic risk associated with retinoids, even in topical form.

Side effects

Common local reactions include skin dryness, peeling, erythema, and mild burning or itching. Prolonged topical antibiotic use may lead to the emergence of erythromycin-resistant Cutibacterium acnes strains. Avoid contact with eyes and mucous membranes.

Warnings

Due to retinoid content, avoid excessive sun exposure and use sunscreen. Topical retinoids may initially worsen acne (purging phase). Prolonged antibiotic monotherapy should be avoided; combination with isotretinoin reduces resistance risk.

How it works

Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding the 50S ribosomal subunit, reducing C. acnes proliferation and inflammation. Isotretinoin, a vitamin A derivative, binds retinoic acid receptors to normalize follicular epithelial differentiation and reduce comedone formation.
