Uses

Psoralen used in photochemotherapy (PUVA) for the treatment of severe psoriasis, vitiligo, and mycosis fungoides that are unresponsive to conventional therapies. It is used in combination with controlled ultraviolet A (UVA) radiation.

Do not use if

Contraindicated in patients with diseases associated with photosensitivity (e.g., lupus erythematosus, porphyria), aphakia, and invasive squamous cell carcinoma. Pregnancy and breastfeeding are contraindications.

Side effects

Common side effects include nausea, erythema, pruritus, and burning. Long-term use increases the risk of photocarcinogenesis (squamous cell carcinoma of the skin). Cataracts may develop with inadequate eye protection.

Warnings

UVA-protective eyewear must be worn during and for 24 hours after treatment to prevent cataract formation. Avoid sun exposure on treatment days. Long-term follow-up is required due to risk of skin cancer.

How it works

Methoxsalen (8-methoxypsoralen) is a photosensitizing agent that intercalates into DNA upon UVA activation, forming monofunctional and bifunctional photoadducts that inhibit DNA synthesis and cell proliferation, reducing the abnormal epidermal turnover seen in psoriasis.
