Uses

A beta-2 adrenergic agonist used as a uterine relaxant (tocolytic) to inhibit preterm labor. It is used to delay delivery in patients with uncomplicated premature labor between 20 and 36 weeks of gestation.

Do not use if

Contraindicated before the 20th week of pregnancy, and in conditions where continuation of pregnancy is hazardous (antepartum hemorrhage, eclampsia, severe pre-eclampsia, intrauterine fetal death, cord compression, placenta praevia). Also contraindicated in patients with cardiac arrhythmias, uncontrolled diabetes, and thyrotoxicosis.

Side effects

Common maternal side effects include tachycardia, palpitations, tremor, nausea, vomiting, headache, and hyperglycemia. Fetal and neonatal tachycardia may occur. Serious adverse effects include pulmonary edema, myocardial ischemia, and severe hypotension.

Warnings

Monitor mother and fetus closely during intravenous administration. Fluid overload and pulmonary edema have been reported. Use with extreme caution in patients with cardiovascular disease. Maternal serum glucose and potassium should be monitored during therapy.

How it works

Ritodrine selectively stimulates beta-2 adrenergic receptors in uterine smooth muscle, activating adenylate cyclase and increasing intracellular cyclic AMP levels. This decreases myometrial contractility and relaxes uterine smooth muscle, thereby inhibiting uterine contractions.

Storage

Store tablets at controlled room temperature, 15°C to 30°C. Protect from light and moisture.
