Uses

Urokinase is a thrombolytic enzyme used for the lysis of acute massive pulmonary emboli, deep vein thrombosis, peripheral arterial occlusion, and clearance of occluded intravenous catheters. It converts plasminogen to plasmin, which degrades fibrin clots.

Do not use if

Contraindicated in active internal bleeding, recent (within 2 months) intracranial or intraspinal surgery or trauma, intracranial neoplasm, severe uncontrolled hypertension, and known hypersensitivity to urokinase.

Side effects

The major adverse effect is bleeding, which can range from minor oozing to life-threatening hemorrhage including intracranial bleeding. Fever, hypersensitivity reactions, and nausea may also occur.

Warnings

Use with extreme caution in any condition where bleeding risk is elevated. Monitor for signs of bleeding throughout therapy. Have blood typing, crossmatching, and coagulation studies available. Concurrent anticoagulants increase bleeding risk.

How it works

Urokinase is a serine protease derived from human renal cells that directly activates plasminogen to plasmin, causing fibrinolysis. Unlike streptokinase, it is not antigenic and does not require a plasminogen activator complex.

Storage

Store refrigerated at 2–8°C. Reconstituted solution should be used immediately or within the time specified by the manufacturer.
