Uses

A proteasome inhibitor indicated for the treatment of adult patients with multiple myeloma and mantle cell lymphoma.

Do not use if

Contraindicated in patients with hypersensitivity to bortezomib, boron, or mannitol. Absolutely contraindicated for intrathecal administration due to fatal outcomes.

Side effects

Common adverse reactions include peripheral neuropathy, fatigue, nausea, diarrhea, constipation, thrombocytopenia, neutropenia, anemia, and herpes zoster reactivation. Serious reactions include hypotension, cardiac toxicity, pulmonary toxicity, and posterior reversible encephalopathy syndrome.

Warnings

Monitor for peripheral neuropathy and adjust dose or discontinue as needed. Use caution in patients with hypotension, pre-existing cardiac disease, or pulmonary conditions. Herpes zoster prophylaxis should be considered.

How it works

Bortezomib reversibly inhibits the chymotrypsin-like activity of the 26S proteasome, disrupting ubiquitin-mediated protein degradation, thereby affecting multiple signaling cascades that regulate cell cycle, apoptosis, and angiogenesis in tumor cells.

Storage

Unopened vials may be stored at controlled room temperature 25°C (77°F), with excursions permitted from 15°C to 30°C. Retain in original packaging to protect from light.
