Uses

The fixed combination of bimatoprost (a prostaglandin analogue) and timolol (a non-selective beta-blocker) is indicated for the reduction of elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension who are insufficiently controlled by monotherapy with either agent.

Do not use if

Contraindicated in patients with reactive airway disease (asthma, severe COPD), sinus bradycardia, second- or third-degree AV block, overt cardiac failure, or hypersensitivity to either component. Use with caution in patients with diabetes due to masking of hypoglycemic symptoms.

Side effects

Ocular side effects include conjunctival hyperemia, eye pruritus, periorbital fat atrophy, iris pigmentation changes, and eyelash growth (hypertrichosis). Systemic absorption of timolol may cause bradycardia, bronchospasm, and hypotension, particularly in susceptible patients.

Warnings

Apply gentle nasolacrimal occlusion after instillation to minimize systemic absorption of timolol. Not recommended for use in children. Monitor for respiratory and cardiac effects, particularly in elderly patients or those with pre-existing cardiopulmonary conditions.

How it works

Bimatoprost is a prostamide that lowers IOP primarily by enhancing uveoscleral and trabecular outflow of aqueous humor. Timolol reduces aqueous humor production by blocking beta-adrenergic receptors in the ciliary body epithelium. The two mechanisms are complementary, providing additive IOP reduction.

Storage

Store at room temperature (15–25°C), protected from light. Discard any unused portion 4 weeks after first opening the container.
