Uses

Bumetanide is a potent loop diuretic indicated for the treatment of edema associated with congestive heart failure, hepatic disease (cirrhosis with ascites), and renal disease including the nephrotic syndrome.

Do not use if

Contraindicated in anuria, electrolyte depletion states (severe hyponatremia, hypokalemia), hepatic coma, and in patients with known hypersensitivity to bumetanide or sulfonamide derivatives.

Side effects

Common adverse reactions include muscle cramps, dizziness, hypotension, headache, nausea, and electrolyte disturbances (hypokalemia, hyponatremia, hypomagnesemia). Hyperuricemia, ototoxicity (particularly at high doses or with concurrent aminoglycosides), and encephalopathy in hepatic disease are recognized risks.

Warnings

A potent diuretic; excessive dosing can cause profound diuresis with severe water and electrolyte depletion. Monitor electrolytes and renal function regularly. Ototoxicity may occur, particularly with rapid intravenous injection or high doses. Use with caution in hepatic cirrhosis.

How it works

Bumetanide inhibits the Na-K-2Cl cotransporter (NKCC2) in the thick ascending limb of the loop of Henle, blocking reabsorption of sodium, chloride, and potassium. Its diuretic potency is approximately 40 times that of furosemide on a milligram basis, with a rapid onset of action.
