Uses

Piperazine theophylline ethanoate (etofylline or etamiphylline) is a xanthine derivative bronchodilator used for the symptomatic relief of bronchospasm in asthma, chronic obstructive pulmonary disease (COPD), and other reversible airways obstruction.

Do not use if

Contraindicated in patients with known hypersensitivity to xanthine derivatives, piperazine, or ethanoate compounds. Use with caution in patients with cardiac arrhythmias, peptic ulcer, seizure disorders, or hepatic impairment.

Side effects

Side effects include nausea, vomiting, headache, tachycardia, palpitations, insomnia, and irritability. At toxic levels, theophylline derivatives may cause arrhythmias and seizures. The piperazine component carries a low risk of neurotoxicity.

Warnings

Monitor for signs of toxicity including nausea, arrhythmias, and agitation, especially in patients with hepatic or cardiac disease. Avoid concurrent use with other xanthine derivatives or medications that inhibit xanthine metabolism.

How it works

Etamiphylline (piperazine theophylline ethanoate) acts primarily as a phosphodiesterase inhibitor, increasing intracellular cyclic AMP. This leads to smooth muscle relaxation in the bronchi, causing bronchodilation. It also has mild stimulant effects on the respiratory center and mild diuretic properties.
