Uses

This fixed-dose combination of glimepiride and metformin is indicated for the management of type 2 diabetes mellitus in adults when diet and exercise alone do not provide adequate glycemic control, particularly when dual therapy with a sulfonylurea and a biguanide is appropriate.

Do not use if

Contraindicated in patients with type 1 diabetes, diabetic ketoacidosis, renal impairment (eGFR below threshold), hepatic dysfunction, and conditions predisposing to lactic acidosis. Contraindicated with hypersensitivity to either component.

Side effects

Common side effects include hypoglycemia (particularly with glimepiride), gastrointestinal disturbances such as nausea, diarrhea, and abdominal discomfort (primarily from metformin). Lactic acidosis is a rare but serious risk with metformin.

Warnings

Risk of hypoglycemia, particularly in elderly patients, those with renal or hepatic impairment, or those skipping meals. Metformin should be withheld before iodinated contrast procedures. Monitor renal function periodically.

How it works

Glimepiride stimulates insulin secretion from pancreatic beta cells by blocking ATP-sensitive potassium channels (sulfonylurea class). Metformin reduces hepatic glucose production and improves peripheral insulin sensitivity via AMPK activation without stimulating insulin release.
