Uses
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus inadequately controlled on metformin alone, or in patients already treated with the combination of vildagliptin and metformin as separate tablets.
Do not use if
Metformin is contraindicated with severe renal impairment (eGFR < 30) and hepatic failure. Vildagliptin is contraindicated with hepatic impairment. Contraindicated in type 1 diabetes.
Side effects
Common side effects include nasopharyngitis, headache, dizziness, nausea, and hypoglycemia (especially when combined with a sulfonylurea). Rare but serious risks include pancreatitis and hepatotoxicity.
Warnings
Monitor liver function before and periodically during treatment due to rare vildagliptin-associated hepatotoxicity. Withhold metformin before iodinated contrast procedures. Risk of pancreatitis; discontinue if suspected.
How it works
Vildagliptin is a DPP-4 inhibitor that increases circulating GLP-1 and GIP levels, enhancing glucose-dependent insulin secretion and suppressing glucagon. Metformin reduces hepatic gluconeogenesis and improves peripheral insulin sensitivity.
Storage
Store at room temperature (15–30°C), in a dry place protected from moisture. Keep in original blister packaging.
